A Note From Our Office

Dear Patients,
We Are Back!
In order to provide you with a safe and convenient patient experience, we have made some changes to our office policies and procedures. Please contact us if you have any questions.

Patient screening – Whether you are a new or current patient if you need an appointment go online at www.dentalplusdc.com. Click on Patient Forms. There, you can update your information and fill out the prescreening form.

All Patients: PLEASE UPDATE YOUR INSURANCE if it has changed! Please note that if you have not done so before your appointment YOU WILL BE CHARGED for your full service on that day. Please email front and back of your insurance card to dentalplus1@optimum.net

At Check-In: Please call us from the parking lot upon arrival. You will then wait in your vehicle until we call you to come in. DO NOT COME TO THE DOOR. Once we call you, please come in alone. You will have your temperature taken and then we will have you wash your hands. We will then advise you to go back to the operatory. Please note that our waiting room is not open. Please wear a mask while you are in the office.

Payments: If you know you have a co-pay please call us the day before so we can process your payment, as we are trying to limit our face-to-face contact. Due to COVID-19 and PPE changes EVERYONE now has a $10 – $20 copay.

Leaving: When you leave, there is no need to stop at the front desk. You can call us to make your follow-up appointment.

If you do not feel well, are running a fever or believe they have been exposed to COVID-19 and are waiting through the two-week incubation period, please do not make an appointment or come to the office.
